Arriving at a doctors office for the first time can be a nerve-racking experience. Paperwork to fill out, new faces and names to learn, and getting to know your way around the office can be overwhelming.

At Fischer Health & Rehabilitation our aim is to  remove any stress from your first visit to our office and make you feel comfortable. In this section of the website you will be able to complete any necessary information prior to your visit.

We encourage you to print out the applicable forms (2) and complete the information at home.  When finished, simply bring the forms with you to your first office visit.  We value your time and strive to make you feel comfortable while you're here.  A staff member will direct you as needed and answer any questions you may have.
